The DD Form 1348-6 (DOD Single Line Item Requisition System
Document)(Figure 35) is used to request non-NSN items, classified items, MWO
and modification kits, and items requiring exception data.
The DD Form
1348-6 is prepared in three copies. Copies 1 and 2 are forwarded to the SSA
and copy 3 is filed in the due-in status.
